INITIAL CONSULTATION
Pro bono teleconference: We offer a free 15 minute telephone consultation with Dr. Hardy as a courtesy. These sessions usually consist of a question and answer session. The purpose of this teleconference is to allow you and Dr. Hardy to determine whether or not Hardy Healthcare is an appropriate practice for your needs.
NEW PATIENTS
New patients must complete our Registration Packet and pay a deposit prior to scheduling your first office visit.
REQUEST A REGISTRATION PACKET
Registration Packets can be requested 24 hours per day online by e-mailing us or by calling the office at (603) 784-5266. Be sure to give us your name, mailing address, telephone number, and fax number or e-mail address if available. Let us know how you would like to receive the Registration Packet – mail, fax or email. We will send you the Packet for you to complete and return to us. All of your information will be kept completely confidential.

SCHEDULING YOUR FIRST APPOINTMENT
Office Consultation: Once a Registration Packet has been completed and the deposit has been received, we will contact you to schedule your first office visit.
FINANCIAL POLICY
Hardy Healthcare operates a fee-for-service practice. Dr. Hardy does not accept any insurance. We will provide you with the forms needed so that you may submit them to your insurance company for possible reimbursement.
